Laparoscopic Inguinal Hernia Repair Symposium

Laparoscopic Hernia Repair is one of the most universally discussed topics of discussion, especially when it involves the chronic pain management aspect post hernia surgery. A laparoscopic hernia repair symposium was organised at BL Kapur Hospital, New Delhi by Aesculap Academy, an academic initiative of B Braun Germany.

Prof Reinhardt Bittner, Buthusa Hernia Clinic, Germany along with Dr Sudhir Kalhan from BL Kapur Hospital performed the hernia repair by the TAPP technique and shared their experiences with regards to chronic pain associated post hernia surgery. Prof Bittner used optilene mesh LP from B Braun and fixed the mesh with Tissuel glue from Baxter. Prof Bittner explained that this new technique of mesh fixation was especially used to reduce the chronic pain post hernia repair.

Many patients suffer from chronic pain and discomfort post hernia surgery. This pain may extend from a few weeks to several months and sometimes indefinitely. There are innumerable causes of chronic pain, however, one of the main reasons could be wrong selection of mesh and nerve damage. Prof Bittner shared his experiences of using a pure polypropylene light weight mesh as an ideal mesh for hernia repair. According to him, after the right selection of the mesh an important role in decreasing the pain associated with hernia repair is also mesh fixation. The mesh can be fixed in many different ways using sutures, tackers, clipping and tissue adhesives. He said that the intensity of the pain is reduced significantly, when the mesh is fixed with fibrin glue. This technique ensures the mesh does not migrate and reduces the chances of pain.

After the surgery, he addressed surgeons at Jaypee Siddharth where he shared his two-decade long experience at Mariam Hospital, Sttutgart. After the meeting, a round table conference moderated by Prof Mishra from AIIMS, New Delhi and Dr Kalhan ensued. The question and answer session proved to be the most important session, where many surgeons talked about the new techniques and experiences in hernia repair trends in India.
