Mumbai-based super-specialty Kohinoor Hospital has recently
performed innovative and advanced spine surgery on four patients suffering from
discogenic low back pain. The surgery- Percutaneous Disc Decompression (PDD)
using Coblation Wand technology is also referred to as nucleoplasty.
Chronic low back pain is the most common ailment in modern
industrial societies. It ranks first among musculo-skeletal disorders, resulting
in serious financial and social consequences, when the discs of the spine enlarge
and start pressing the nerves (herniated disc), which causes severe back pain.
Surgical treatment of intervertebral disc herniation such as open discectomy,
microdiscectomy, and laminectomy are often targeted for patients with uncontained
or large herniations, and/or sequestered discs.
PDD using Coblation technology is an effective procedure for patients presenting
with discogenic back and/or leg pain who have failed conservative therapies
and are not considered candidates for open surgical interventions. In the PDD
using Coblation Technology, a cobaltion wand is inserted in the nucleus of disc.
The wand then imparts heat of 40°C to 70°C to the surrounding area,
shrinking the herniated disc and gives relief from low back pain.
Dr Vishwa-nathan Iyer, Spine Surgeon, Kohinoor Spine Centre, performed these
surgeries with international faculty from Malaysia, Dr MK Radhakrishnan. Both
are specialised in doing PDD with coblation wand technology. Dr Iyer said, "This
is an ultimate form of minimally invasive spine surgery. It is a day care surgery
performed under local anesthesia. Patient gets immediate pain relief and may
go home same day and can resume work soon."
Treatment of back pain by primary care providers typically involves prescription
of opioids, expensive non-steroidal anti-inflammatory drugs (NSAIDs), or physical
therapy. Though these are common treatments, they may not be the optimal solution.
Opioids may be addictive and patients are likely to build drug tolerances.
NSAIDs have potentially dangerous side-effects, and physical therapy may be
ineffective. Many patients suffering with discogenic pain become refractory
to medical management after some time. Minimally invasive techniques addressing
the discogenic pain should therefore be made available to these patients,"
said Dr Iyer.
"Kohinoor Hospitals will play a leading role in expanding the horizons
in advancements of various super specialties, not only with a focus to make
these advancements more patient friendly but also more affordable. Our beginning
of these super specialty surgery within one month of launch reflects the pace
that we are setting in reaching these advancements to our patients," said
Shailesh Dalvie, CEO, Kohinoor Hospitals.
