Apollo HomeCare and Apollo AyurVAID collaborate to strengthen post-discharge transition care

Apollo HomeCare and Apollo AyurVAID have joined hands to address the gap patients face between discharge and returning home safely. To bridge this gap, the two have launched a transition care service, offering patients access to a clinically monitored, in-facility step-down care environment where they are assessed, stabilised, and prepared for a safe return home.

The facilities function as an intermediate care ecosystem between the hospital and home. At the facility, patients continue to receive care based on their condition, including nursing support, physiotherapy, rehabilitation, nutritional guidance, and selective Ayurvedic therapies depending on their stage of recovery. The focus is on ensuring that patients are stable and prepared to return home before discharge. Doctors and care teams from both organisations work together on the ground, monitoring each patient’s progress and responding if there are changes in their condition.

A key feature of this collaboration is the joint clinical approach. Patients enrolled through either healthcare service undergo a combined evaluation by specialists from both systems of care. This shared assessment helps create a personalised transition care plan for each patient that integrates modern clinical medicine with selective Ayurvedic therapies where appropriate. The transition care service is currently operational in Bengaluru, Chennai, and Delhi with multiple beds available.

The service is designed not only for patients based locally in these cities, but also for those who travel from other states and countries for major treatment procedures. This integrated care system provides a monitored step-down phase that allows these patients to stabilise before undertaking travel, helping reduce the clinical risks associated with early discharge and long-distance movement during recovery.

Apollo HomeCare’s experience in post-discharge clinical care combines with Apollo AyurVAID’s NABH-accredited facilities and protocols for clinical documentation and outcome tracking. Together, they form a care partnership focused on continuity of care during recovery.
